The Health Status Box displays the following information for MyChart users: : Advanced Directive, Alerts, Allergies, Blood Type, Emergency Contact, Organ Donor and Physician Contact information. The Health Status Box can be found at the top of every MyChart page.
All information that is displayed in the Health Status Box is self inputted by the user.
The Health Status Box can be accessed by clicking on My Health Record or by clicking the emergency information link located at the top of every MyChart page under the Patient's name.
When you enter information in your Personal Health Record for Allergies and Conditions and mark them as "Active" the information will display in Allergies and Alerts in your Health Status Box.
When information is entered in "Important Clinical Information" for Blood type, organ donor and advanced directive the information entered will display in the Health Status Box.
By selecting ''Other Contacts" under the Contact tab and entering contact information marking them as emergency contact or emergency physician will display the information in the Health Status Box in the appropriate categories.
To change or edit information in your Health Status Box, you will need to access Personal Health Record and select the category you would like to change. Marking the condition or allergy as "inactive" will remove it from the Health Status Box.
To make changes to your emergency contact or physician information, you will need to access Manage My Account, select the contact you would like to change and make the necessary changes.
