In order to register for MyChart, you need a PIN. A PIN is a unique 4 digit number assigned to you by the MyChart support team.
Please do not share your PIN with anyone, except for registration purposes.
It is a one-time-use registration number that must be validated before you can register a username and password for your account.
If you have not been assigned a PIN via e-mail, one can be generated in person after presenting your OHIP card at one of our offices below:
- Ambulatory Care Centre (ACC) Patient Registration Desk - located just inside the Gilgan entrance on the Ground Floor of the Main Hospital building in the Glendale wing.
- Health Records Department - located on the Ground Floor of the Main Hospital building in the Morrow wing (beside the main elevators).
Once you have received your PIN, fill in all the fields on the right exactly as they are shown on your Ontario Health Card.
Fill in your First and Last Name as recorded on file at St. Joseph's Health Centre (Toronto).
