In order to register for MyChart, you need a PIN. A PIN is a unique 4 digit number assigned to patients by the MyChart support team. Please do not share this PIN with anyone, except for registration purposes. It is a one-time-use registration number that must be validated before you can register a username and password for your account.
If you have not been assigned a PIN via e-mail, you can register by providing patient registration clerks with their proof of identify (Ontario health card with photo) along with their email address. Once the email address is collected at West Park, the patient will receive an e-mail with a confidential PIN that allows them to complete registration on the MyChart website.
If you are setting up an account on behalf of a patient, please ensure you bring either a) a power of attorney document or b) a completed delegate form along with a copy of the patient’s ID and also photo ID of the delegate (available through patient registration clerks).
Please fill in all the values exactly as they shown on OHIP card. Please note that Ontario Health Card Number field accepts numeric values only.
