You will be required to obtain a PIN in order to register an account. A PIN can be obtained in the following ways:

Option 1: Instantly
At the time of registration at the hospital, with a Registration Clerk or at a kiosk, opt-in to receive a PIN instantly. Use the MyChart registration page with your unique PIN to enrol using your unique PIN on your smartphone.

Option 2: Within 24 - 48 hours
At a later date or without a MyChart PIN page from registering at Headwaters, a PIN can be obtained in the following ways:

• Online at
  www.headwatershealth.ca/mychart
• By Phone at (519) 941.2410 x2208
• At Health Records located in the lower level of the hospital
• If you are setting up an account on behalf of a patient, please ensure you bring either
  
  1. a power of attorney document or
  2. a completed Activation and Consent form along with a copy of the patient's ID and also photo ID of the delegate
